Jio-Bio-Care Proposal Memo

The frame and the organizing idea

Three things have become true at once. Jio Allianz is months from a launch date that gives us a captive payer of national scale. The largest block of private insurance claims in India is cardiometabolic disease and its hospital-borne complications, most of which is preventable with a ten-year lead time if someone can see the trajectory. And the unit cost of deep phenotyping has fallen far enough to make population-scale biobanking economic for the first time in any market. The window to build the defining health franchise in India is the next twenty-four months. This memo proposes what to build, why only we can build it, and what to decide first.

What is not the problem. Primary care in India is not scarce. Pharmacies, clinics(licensed and unlicensed), diagnostic chains, and telehealth saturate every postcode. Patients route themselves to whoever is closest and fastest when symptomatic, and they are largely satisfied with the relief they receive. Adding more primary care capacity does not guarantee usage nor change outcomes, does not shift claims, and does not build a franchise.

What is the problem. Two things break in Indian private health, and both are addressable at our scale. The first is that the relationship between a member and a health system is transactional and episodic. People meet their clinician in the crisis, not before. The second is that secondary and tertiary care are navigated alone. Indian families facing a cardiac scare, a cancer diagnosis, a recommended surgery, or a new chronic diagnosis enter a system that over-tests, steers to higher-margin procedures, charges opaquely, and releases them without follow-up. This is where private insurance claims leak, and this is where the human cost concentrates.

What we are building. A health operating system. The organizing idea is not a clinic chain and not a new insurance product. Jio Allianz is the insurance product. What sits beneath it, and makes it economic, is an operating layer with six interlocking components: a daily engagement stack, a human navigation layer, an intervention-budget engine, a biobank with retrospective assay capability, a Research Trust holding de-identified data, and an insurance interface feeding outcomes back into premium pricing. The experience that runs on top of the operating system is a trusted relationship formed with the member in their healthy years, monetized when they need to navigate secondary and tertiary care, and compounded across their lifetime into the most valuable longitudinal health dataset in the country.

The unit of value that compounds. Revenue, clinic footprints, and premium products can all be copied by any well-capitalized competitor. One asset cannot be copied at our scale within a decade: patient-years of consented, longitudinal, multi-omic phenotype linked to navigation decisions and outcomes. Every architectural choice that follows is in service of compounding this asset faster than any other actor in India, or in fact globally outside the Nordic and UK biobank consortia. Trust is the entry. Data is the flywheel. Outcomes are the proof.

Why us. The specific infrastructure needed to compound this asset already exists in the group. Jio digital identity reaches four hundred and fifty million Indians. JioFiber and JioPhone can enable ambient sensing at household scale as technologies mature. Jio Allianz gives us a captive payer and, through any group policy we choose to hold, a proof vehicle before Allianz is live. Reliance Foundation and Strand Life Sciences anchor the tertiary and omics layers. Jio Brain provides inference. India’s ABDM and DPDPA frameworks give us a consent rail native to domestic law that no foreign actor can replicate. No pure-commercial competitor can assemble this combination in under a decade, and no state-led actor will. The window opens now because assay costs have crossed a line and Allianz is ready.

The social contract. The founding story of this company was trust offered by ordinary Indians in return for industrial dividends. The new story asks the same ordinary Indians to trust us with their health data and their navigation choices in return for better outcomes and a dividend on discovery. The same civilizational contract, in a new asset class. A pharmaceutical firm cannot credibly offer this, because it is a buyer of the data. A hospital chain cannot offer this, because it is a seller of procedures. A foreign actor cannot offer this, because it cannot promise return of value. Only we can.

The architecture, the build options, the commercial engine

The operating system. Six components, each load-bearing and each designed to plug into the others.

Daily engagement is the surface the member sees every day. A Personal Agent lives on the member’s device and holds their personally identifiable information locally. A Clinical AI inference layer processes consented data on demand and retains nothing; it is stateless by design, so the compute layer can never become a honeypot. A community health worker cadre at a density of one per thousand members provides the human escalation path when the agent judges that human contact is needed. This is where trust is formed.

Navigation is the layer that activates when a clinical signal appears or an event looms. A human navigator, drawn from the CHW cadre and a specialist coordinator team, walks the member through their secondary or tertiary care. They recommend the hospital and the specialist. They flag over-testing and over-procedure. They negotiate pricing. They compress admission duration. They close the loop on discharge, adherence, and chronic follow-up. This is where trust is monetized.

The intervention-budget engine disciplines the care side by the insurer’s profit and loss. Every clinical action, from a remote consultation to a recommended procedure, runs against an eighteen-month payback loop. Interventions that do not demonstrably reduce claims are retired. This is how care becomes economic rather than volume-driven.

The biobank, built with retrospective assay as a first principle, collects biosamples from every member at enrollment. Assays run only on the subset triggered by clinical or research relevance. This is how deep phenotyping moves from a few lakhs per participant upfront to tens of thousands at steady state.

The Research Trust is a separate legal entity. It holds de-identified data and biobank custody and signs pharma and academic partnerships without contaminating the insurer’s or the care entity’s profit and loss. It is also the channel through which the Data Dividend flows back to members.

The insurance interface is the feedback loop between outcomes and premium pricing. Jio Allianz is the anchor buyer. Any Indian group insurer is a sufficient proof vehicle in the interim.

Primary care lives inside the operating system but is not the product. Since primary care is already supplied by the Indian market at density and price, our primary care presence is the Personal Agent, the CHW cadre, and a thin physical footprint where in-person touch is clinically material. Physical infrastructure is data capture and trust maintenance. It is not a retail business and will not be sized as one.

The build options. Four ways to assemble the operating system exist, and the decision between them is the most material one we face after the framing itself. An asset-light platform owns data, agent, navigator, and Research Trust and partners everything clinical. An asset-medium build owns the data spine and a thin physical footprint and partners labs, primary care, and hospitals. An asset-heavy build owns the delivery chain end-to-end, at the capex and complexity implied. A platform-as-a-service exposes the operating system to third-party insurers, employers, and hospital chains with the thinnest direct consumer presence. Each of the internal plans in circulation maps onto one of these options. The position of this memo is that these are options to be chosen after ninety days of validation, not commitments to be made today.

The commercial engine. Care does not need to pay for itself in Year One. Two revenue streams pay for it earlier than care economics can on their own, and both are material.

Navigation-linked margin capture. The largest and most opaque block of private insurance claims in India is secondary and tertiary care. A trusted navigator reduces unnecessary procedures, routes to better-performing hospitals, negotiates pricing, compresses admissions, and closes the discharge loop. On a hundred-thousand-member book, partial conversion of these levers is worth several hundred crores a year in reduced claims. The revenue can be booked two ways: directly from the member as a subscription that bundles the navigator, and from the insurer as a per-member-per-month management fee or a share of the claims savings demonstrably produced. Both forms activate the same underlying mechanism.

Research partnership revenue. The Research Trust holds four assets with distinct revenue shapes: a metabolomics reference panel, a proteomics and pQTL panel, a CGM glycaemic reference dataset, and — over a longer horizon — a pre-disease trajectory biomarker. Comparable deals establish that the category is real and the magnitudes are material: pharma partners in the UK Biobank proteomics consortium at forty to one-hundred-twenty crores each, deCODE’s Roche and Amgen arrangements in the low thousands of crores, 23andMe’s GSK partnership at the twenty-five-hundred-crore mark, and biomarker-licensing valuations in the tens of thousands of crores at the Grail precedent. The ninety-day sprint will size each asset bottom-up against Indian pharma willingness-to-pay and return with defensible ranges. The Year One milestone is LOIs signed in at least two of these shapes; the Year Four milestone is conversion to revenue. Neither is speculative — both are anchored in deal shapes the global biobank category has already demonstrated.

Capex and the honest FCF story. The ninety-day sprint is small: a focused team, LOI conversations, a unified specification. The 180-day consumer pilot is mid-scale: five hundred to one thousand members, Personal Agent MVP, navigator cadre, biobank seed, assay float. The Year Two scale-up is material but gated on pilot outcomes. The FCF-by-Year-Three ambition is achievable. It is not deliverable from primary care margin, because Indian primary care does not turn free cash flow positive in three years and we are not in that business. The honest path to Year Three free cash flow is the research partnership ladder converting LOIs to signed deals across Years One to Three, combined with navigation-linked claims savings from Year Two onward. The sprint’s first deliverable is hard numbers against each of these.

The proof, the phasing, the ask

The minimum viable proof. A single cohort of members, run on the full architecture, measured against five legs simultaneously. First, members enroll and engage in the healthy/non-clinical state; trust forms. Second, when clinical events occur, members route through the JioCare navigator rather than around us; trust is operational. Third, deep phenotype produces clinically meaningful risk stratification that changes the timing or the choice of intervention. Fourth, outcomes translate to measurable claims-cost reduction priced by an insurer, whether Jio Allianz when it is live or a group insurer before then. Fifth, de-identified data finds buyers; at least two pharma LOIs are signed inside the first hundred and eighty days. All five in one pilot, not in series. Any one leg failing tells us which piece is weak before we scale.

The first customer - deliberately open. Three beachheads are credible and each produces a different economic and data shape. Urban affluent, forty-five to sixty-five, cardiometabolic: a clinical-grade product at three to four lakhs a year, where trust forms through preventive surveillance and navigation monetizes fast because secondary events are near. Urban affluent, thirty to forty-five, performance and longevity: a consumer-grade product at seventy-five thousand to one and a half lakhs a year, where trust forms through daily insights and ambient sensing, and the compounding relationship is the longest of the three. Employer groups, twenty-five to fifty-five: a capitated group policy, where the employer buys trust on behalf of the employee and the scale ramp is the fastest. The same Personal Agent, the same navigator, the same biobank, the same Research Trust, and the same Data Dividend underlie all three. Pre-deciding in April is a bet that should be a decision in July, with the economics and the signed LOIs in hand. The sprint’s output is a chosen beachhead priced and ready to launch, not three candidates and a recommendation.

Phasing. Three gates.

Gate 1, end of July 2026: a fundable plan. A unified specification across all six operating system blocks. Economics replacing every placeholder. The Research Trust legal structure filed. Two to three signed pharma LOIs. A chosen beachhead with pilot design. A chosen build option.

Gate 2, end of October 2026: a consumer pilot live in the chosen beachhead. Five hundred to one thousand members on the full architecture, including the navigation layer. Six months of engagement, navigation, outcomes, and biobank seed in hand by March 2027.

Gate 3, mid-2027: the Jio Allianz-wrapped cohort launches. The scale decision is taken with twelve months of outcomes and signed pharma partnerships already on record.

The ask. Not capital today. Four things. One, endorse the framing: the operating system is what we are building; trust built before the event and navigation at the event are what the member experiences and pays for; data compounding is the moat; the Data Dividend is the contract. Two, name one owner for the operating system, reporting into your office. Three, authorize a ninety-day sprint at a small funded scope. Four, park the distribution plan debates until the sprint concludes; every one of them plugs into the operating system, and none should be promoted before the sprint’s economics are in hand. We return by the end of July 2026 with economics that can be priced, a chosen build option with a committed owner, a consumer pilot ready to go live in October, and the first pharma LOIs on the table.

Intent:

We believe that key modalities of healthcare delivery and organization can, will and should change in the next 10 years. We are building the infrastructure that drives this change. A healthcare for every Indian that is data and evidence based, abundant, preventative and customer-centric.

Insights from Prior Experience:

  1. Abandon Industrial Revolution Healthcare: Our hospital centric system was built for an acute disease burden, which has inverted to chronic (70% of the burden today). The hospital center model was built when both the technology and the knowledge was centralized (i.e., in the walls of the building and in the physician’s head). Chronic needs to be managed differently (30-40% clinical and 60% environmental, behavioral and social drivers). jalanishaan@gmail.comBroken incentives?

  2. AI inference costs collapse the supply constraint: India has 70 doctors per 100,000 people; 60% of primary-care interactions in low-income India are with unqualified providers. Genome sequencing fell from $3B in 2003 to under $100 today — faster than Moore’s Law for 15 years. Clinical AI inference sits on the same trajectory: ~$100/member/year today, $10–15 within four years. This enables 80% task-shifting to AI + community health workers. The window during which depth, data, AI, and regulation combine into a structural moat is short.

  3. Not all data is equal: Longitudinal data is specifically what we need to capture. We fundamentally believe that Longitudinal biological and medical data is uniquely valuable because it captures how health changes over time—revealing trajectory, response, and progression that static cross sectional datasets cannot. That makes it especially powerful for prediction, precision care, and therapeutic discovery when linked across rich clinical and molecular data. This is what will underpin both a bio and a personalized medicine strategy and development for us.

  4. India is the world’s most valuable un-sequenced biological population.

    1.4 billion people. 4,600 endogamous population groups. 7 million novel genetic variants. Almost none of this exists in the global research base. Indian patients receive treatments designed for European genomes. Pharma that wants to develop for South Asian disease biology cannot do it today. India can run trials at 1/10th US cost with 10x faster enrollment. A pre-consented, genotyped, continuously-monitored JioCare cohort is a structural asset that cannot be replicated.

  5. Capturing Longitudinal Data: Longitudinal data is valuable but operationally hard to obtain at scale—retention, consistent testing, and cost are the barriers. The largest longitudinal study today in India is the LASI study with 72,000 in phase 1. The largest globally are the All of Us (US) and UK Biobank (UK) with 1 million and 500K participants respectively. To obtain this efficiently, it must be captured as a by-product of a long term, high retention customer relationship. This is why we play a key role in directly providing care.

  6. No fixed healthcare supply: While we want to provide care, we will focus on building intelligence and flexible supply, not clinics or hospitals. Return-on-asset math corrupts clinical decisions and makes one focus on volume; we refuse that structure. Less than 15% of total capex in our plan will be on clinics, buildings, and/or pods.

  7. Trust at the individual level is a pre-condition to providing care: Trust in preventative healthcare is produced by witnessed commitment rather than by formal authority or brand alone. The implication of this is that we believe that a model where a live person owns and builds the relationship with a patient will have higher retention, better care outcomes, and create more opportunities for data capture.

  8. We will build a new healthcare workforce around the frontline worker going to the home: The core instrument is the community health worker, augmented by a 24/7 AI doctor in 8+ Indian languages, ambient monitoring through existing Jio devices, and point-of-care diagnostics. A pod sees 38 symptomatic walk-ins per day and misses 75% of the catchment. Half of India’s hypertensives are undiagnosed; they will not walk into a pod. CHW + AI reaches every home. This personal relationship is key to building trust and creating locally specific models of care. This also solves the other key challenge - demand for primary care is extremely challenged and any inconvenience or friction dramatically suppresses it.

The Operating Model

Specification of the five systems — Access, Escalation, Underwriting*, Data, Science.

This document specifies Jio Arogya as a register of components. Each component is defined by a specification table describing its function, scope, inputs, outputs, exclusions, and build requirements. Summary matrices show how the components relate to one another. The Access, Underwriting, and Science systems follow a uniform specification form. The Escalation system uses a cross-tier matrix because the controlling question is how four contract types substitute for hospital ownership. The Data system uses instrumentation and signal registers because the controlling question is what is captured, how often, and what longitudinal record accumulates.

*This includes underwriting, though it would need to be workshopped with the Jio Alliance team in collaboration.

1 · Underwriting

Jio Allianz carries the risk. Jio Care delivers the care. Plans are co-designed.

Premium, reserves, claims, and grievance redressal sit on the Jio Allianz balance sheet under the IRDAI licence. Clinical care sits in Jio Care under its protocols and ethics board. Plans are priced jointly: the intervention budget for each plan is computed from the actuarial baseline and released to care delivery as the operating envelope. The two entities are legally separated. Data flow between them is DPDP-compliant, consent-ledger-tracked, and purpose-limited.

Three components reach the member. Engagement is ordered.

The access system contains three operating components: a per-member conversational interface running on the member device, a stateless central clinical inference service, and a distributed cadre of salaried Community Health Workers. Each component has a defined scope, deployment footprint, and handoff rule. Cross-component flow is governed by the Access Rule (Component 1.4).

2.4 · Access Rule  
Statement Layer 1 (Personal Agent) answers first. Layer 2 (Clinical AI) answers the subset of queries that Layer 1 escalates. Layer 3 (Frontline Worker) intervenes only on Layer 2 escalation or on a scheduled protocol visit. Member self-dispatch of the CHW is not permitted.
Logging Every escalation carries a structured reason code. Every physical visit carries a clinical trigger recorded against the member’s longitudinal record.
Rationale • The CHW minute is the most expensive unit in the access stack • Self-dispatch would saturate CHW capacity and breach the intervention budget • Ordered engagement is required for the plan-level unit economics to close
Audit Rule enforcement is audited weekly by the actuarial loop (Component 3.4).

3 · Escalation

Four contract tiers substitute for hospital and clinic ownership.

Hospital and clinic ownership would solve four problems: control over the episode, cost of the stay, visibility into outcomes, and owning the patient relationship. However, owning and running hospitals is asset heavy, creates the wrong (volume) incentives and distracts from the real value add work. So we propose that instead of owning hospitals and large clinic chains, we substitute for four contract types. Almost every clinic and hospital in India is placed in one of four tiers. The matrix below specifies how each tier addresses each of the four problems; the specification tables that follow give contract-level detail.

Operating principle. Jio Arogya does not operate hospitals. Jio Arogya operates the layer that knows the member, orchestrates the stay, captures the data, governs the outcome, and retains the relationship before, during, and after the admission. Hospital ownership is not the unit of control; contract design plus data integration plus patient-side services is.

4 · Data

What we capture, through what instruments, at what frequency, and what longitudinal record accumulates.

Intelligence has a shelf life. Static datasets devalue; trained models age; continuous instrumentation compounds. The data system is therefore specified in two registers. The Instrumentation Register (Component 4.1) enumerates every sensor and feed, what it captures, at what frequency, and whether it is existing or built. The Longitudinal Record (Components 4.2 and 4.3) specifies the dataset that accumulates per member per year and across the full five-year enrolment. Architecture, consent, and the Data Dividend are specified in Components 4.4 and 4.5.

Privacy is enforced in infrastructure rather than in policy. The architecture consists of three layers across three legal entities, with three boundaries. Every cross-boundary event is logged and audited. The architecture is ABDM-native: ABHA ID is the member identity, the ABDM Consent Manager is the legal spine, the Health Claims Exchange is the claims rail, and Health Locker is the member’s portable record.

5 · Science

Continuous instrumentation generates the substrate. Science converts it into new medicine.

The Research Trust holds the research-consented dataset and operates the scientific programme. Four components define the programme: an autonomous research engine (5.1), a maturity-graded study ladder (5.2), a biobank of physical samples (5.3), and a governance structure holding the ethics, access, and licensing rules (5.4).

5.1 · AI Scientist  
Function Autonomous research system operating on the Research Trust’s de-identified dataset. Generates hypotheses, designs cohort studies, and runs primary analyses. Human scientists review, validate, replicate, and publish.
Operating principle Compresses the research loop from the conventional five-year design-collect-publish cycle to a six-week query-validate-publish cycle against the live cohort. This is the operational answer to the shelf-life-of-intelligence thesis: research must run at the pace at which intelligence itself ages.
